The Postpartum Social Support Screening Tool

In 2013, The Postpartum Social Support Screening Tool was adapted by The Artemis Center for Guidance, LLC ™ from a questionnaire developed by the Advancing Diabetes Self-Management project at La Clinica de La Raza, Inc. in Oakland, CA with support from the Robert Wood Johnson Foundation® in Princeton, NJ.

We utilize this tool to help women and families identify the various forms of social support in their system. We use it as a treatment planning tool to assist women in strengthening their support system. It can create a powerful dialogue where women and families are empowered to strengthen their resource and use them to their fullest potential.
